സർക്കാർ ജീവനക്കാരുടെ ആരോഗ്യ ഇൻഷുറൻസ്
പദ്ധതിയായ MEDISEP ൽ ഓണ്‍ലൈനായി രജിസ്റ്റര്‍ ചെയ്യുന്നതിനുള്ള സംവിധാനം ചില സാങ്കേതിക ബുദ്ധിമുട്ടുകള്‍ കാരണം തത്കാലം നിര്‍ത്തി വെച്ചു. അതിന് പകരമായി ജീവനക്കാര്‍ ഒരു പ്രൊഫോര്‍മ സ്ഥാപന മേധാവിക്ക് പൂരിപ്പിച്ചു നല്‍കുകയും സ്ഥാപനമേധാവി ഇതിന്‍റെ കണ്‍സോളിഡേറ്റഡ് സ്റ്റേറ്റ്മെന്‍റ് നിശ്ചിത മാതൃകയിലുള്ള എക്സല്‍ ഫയലില്‍ തയ്യാറാക്കി അതത് വകുപ്പുകള്‍ നിയമിക്കുന്ന നോഡല്‍ ഓഫീസര്‍മാര്‍ക്ക് കൈമാറുകയും ചെയ്യണം. ഇതിനകം MEDISEP പോർട്ടലിൽ ഓൺലൈനായി വിവരങ്ങള്‍ നൽകിയവരും ഒരിക്കൽ
കൂടി പ്രൊഫോർമ വഴി വിവരങ്ങൾ സ്ഥാപന മേലധികാരിക്ക് നൽകേണ്ടതാണ്. വിവരങ്ങൾ
കൈമാറേണ്ട അവസാനതീയതി : 2018 സെപ്റ്റംബർ 29.

ഷീറ്റ് തുടങ്ങിയവ താഴെ ലിങ്കില്‍ നിന്നും ഡൗണ്‍ലോഡ് ചെയ്യാവുന്നതാണ്

ഇതുമായി ബന്ധപ്പെട്ട സംശയനിവാരണത്തിനായി 0471 230 5851 എന്ന നമ്പറിൽ ബന്ധപ്പെടാം.
ആരോഗ്യ ഇൻഷ്വറൻസ് പദ്ധതിയിൽ ഉൾപ്പെടുത്തിയവർ
ജീവനക്കാർക്കു പുറമേ
1. ഭർത്താവ്/ഭാര്യ
2. മകൻ/ മകൾ (25 വയസ് പൂർത്തിയാകുന്നതുവരെയോ അല്ലെങ്കിൽ വിവാഹം കഴിക്കുകയോ അല്ലെങ്കിൽ ജോലി ലഭിക്കും വരെയോ)
3.ജീവനക്കാരെ മാത്രം ആശ്രയിച്ചു കഴിയുന്ന മാതാവ്, പിതാവ്
4.ശാരീരിക/മാനസിക വൈകല്യമുള്ള കുട്ടികൾ.(ഇവർക്ക് പ്രായപരിധി ബാധകമല്ല)
